- Provide consultation to prospective food facility and public swimming pool operators
Plan Submittals
We accept paper and electronic plans. Paper plans can be submitted at our San Bernardino, Rancho Cucamonga and Hesperia offices. Assistance with submittals and payment is available at the Rancho Cucamonga and Hesperia offices via our virtual assistant, Alice.
Electronic plans can be emailed to EHS.CustomerService@dph.sbcounty.gov. Plans must be attached to the email in pdf format or accessible for download via link to a file-sharing service (e.g. Dropbox, etc.). Starting July 1st, 2020, electronic submittals are eligible for a 5% discount.
Please provide a completed “Application for Plan Review” with paper and electronic submittals. Submittals may be subject to a preliminary review prior to intake. For all inquires, please contact Environmental Health Services.
We encourage applicants to use the “Preliminary Food Facility Checklist – Food Handling” and the “Preliminary Food Facility Checklist – Prepackaged” as a checklist to ensure that plans include the required information. Using the sheet will facilitate plan review, may reduce or eliminate the need for additional submittals and can prevent unnecessary delays in the review process.
